• 5:15 p.m. Tuesday - Nicole Anne Ludwig, 35, of 63 EMS T16 Lane, Leesburg, arrested for felony failure to appear. No bond set.
• 5:21 p.m. Tuesday - Carmen Carolina Garcia, 26, of 1692 E. CR 200N, #40, Warsaw, arrested on a warrant. No bond set.

• 5:23 p.m. Tuesday - Intersection of East CR 200N and North Biomet Drive, Warsaw. Two-car collision. Drivers: Ryan S. Helser, 47, Glad Street, Warsaw; Jayme L. Hawkins, 32, East Springfield Drive, Warsaw. Damage: $2,500-$5,000. Warsaw Police Department investigated.
• 7:33 p.m. Tuesday - Intersection of U.S. 30 and Meijer Drive, Warsaw. Truck-SUV collision. Drivers: Dean W. Schmidt, 57, Des Plaines, Ill.; Jennifer L. Alley, 43, Rochester. Damage: $5,000-$10,000. WPD.
• 8:43 p.m. Tuesday - East CR 700N, 100 feet south of North Harper Road, Leesburg. Van hit tree. Driver: Jeremy J. Crouse, 36, East Prairie Street, Leesburg. Damage: $10,000-$25,000. Kosciusko County Sheriff’s Office investigated.
