• 2:15 p.m. Monday - Zane Corwin Reed, 25, of 202 S. Sherman St., Warsaw, arrested for failure to appear. Bond: $500.
• 8:12 a.m. Tuesday - Philip Gregory Meade, 73, of 2575 E. Center St., Warsaw, arrested on two charges of felony failure to appear. No bond set.

• 7:34 a.m. Friday - Intersection of North Ind. 15 and West CR 400N, Warsaw. Car-pickup collision. Drivers: Jason Velazquez, 17, North Old Ind. 15, Warsaw; Scott Vanwesten, 37, Levi Lee Road, Warsaw. Injuries: Velazquez complained of hip/upper leg pain; a passenger in Velazquez’s vehicle, Jan Carlos, 14, North Old Ind. 15, Warsaw, complained of abdomen/pelvis pain. Damage: $10,000-$25,000. Kosciusko County Sheriff’s Office investigated.

• 3:01 p.m. Friday - EMS B33A Lane, 600 feet north of EMS B33A1 Lane, Warsaw. Parcel delivery vehicle hit cable line. Driver: Michael Rhodes, 45, Kalamazoo, Mich. Damage: $2,500-$5,000. KCSO.
• 11:30 a.m. Saturday - Intersection of East CR 1200N and North Kern Drive, Syracuse. Pickup-truck collision. Drivers: Christopher Young, 32, Ligonier; Mary Sholtey, 45, North CR 350E, Syracuse. Damage: $5,000-$10,000. KCSO.
• 7:38 a.m. Sunday - Intersection of East High and Prairie streets, Atwood. Two-car collision. Drivers: Paul D. Grove, 53, Elkhart; Neil D. Bammerlin, 60, West CR 100S, Warsaw. Damage: $10,000-$25,000. KCSO.
• 5:46 a.m. Monday - Intersection of North Turkey Creek Road and East CR 1000N, Syracuse. Car hit deer. Driver: Sirenna Tom, 25, North CR 1200W, Cromwell. Damage: $2,500-$5,000. KCSO.
