Sheriff’s Office Announces Developmental Disabilities Registration Form For First RespondersWith Autism Acceptance Month beginning Thursday, the Kosciusko County Sheriff’s Office has announced the release of a new Developmental Disabilities Registration Form for first responders.

This project is not exclusive to Autism Spectrum Disorders and can be used for any applicable disability, according to a news release from the KCSO.

The form is available for download from the bottom of the KCSO homepage at www.in.gov/sheriffs/kosciusko.

The return instructions are located in the final paragraph of the documentation.

Once the PDF fillable form has been submitted to the communications center, it will be attached to readily accessible name and address records. Users may also send a photograph of the individual, if they choose.

The purpose of this new feature is to aid responding personnel with potentially critical details in the event of an emergency.
