David A. Perez, 52, Warsaw, formerly of Ligonier and Salina, Kan., passed away at 6:20 p.m. Sept. 6, 2015, at Parkview Regional Medical Center, Fort Wayne. He was born Sept. 30, 1962, in Columbus, Ga., to Andrew and Dorothy Rice Perez. He spent most of his life in Salina. He was an avid fan of the Kansas City Chiefs. Survivors are: two sons, Brian (and spouse Anni) Perez and Nathan Perez, both of Warsaw; two sisters, Carmen Siemsen and Cathy Leonard, both of Salina; a brother, Bill Perez, Salina; and a grandson, Allen Perez, Warsaw. He was preceded in death by his parents and a son. There will be private calling and service at a later date.
