One person was injured after a five-vehicle accident Wednesday at the intersection of West U.S. 30 and CR 150W, Warsaw.
According to the Warsaw Police Department accident report, Jesse R. Cookson, 27, Huntington, said he was travelling east on U.S. 30 in a 2016 Chevrolet. He said he had been driving all day. When he was approaching the intersection with CR 150W, the last thing he remembered was driving 60 miles per hour and seeing white after that. He said he didn’t remember the impact.
According to the accident report, Cookson appeared dazed and complained of minor head pain and was transported to the hospital.
Lucero R. Vazquez, 37, Fort Wayne, said she stopped at the red stop light at the intersection behind a vehicle driven by Jose Ortega Urdanta, 41, Kissimmee, Fla. Vazquez said she was waiting on the light to turn green when she was rear-ended by Cookson’s vehicle. Due to the impact, Vazquez’s vehicle collided with Urdanta’s vehicle.
Angela M. Price, 45, West CR 200N, Warsaw, said she was stopped at the red light behind a vehicle driven by Katlin M. Johnson, 23, Plymouth, and next to Vazquez’s vehicle. Price said she was not sure what happened, according to the accident report. She said she remembered getting rear-ended. After Price’s vehicle was hit, Price’s vehicle rear-ended Johnson’s vehicle.
Johnson said she was stopped at the red light at the intersection. She said she remembers hearing the impact of Vazquez’s vehicle and then having Johnson’s vehicle bumped by Price’s vehicle.
Urdanta said he was stopped at the light when he was rear-ended by Vazquez’s vehicle.
Estimated damage to all vehicles was $25,000 to $50,000.
