Bobby Darrell Lee Jr., whose mailing address is 305 S Clay St., Claypool, IN 46510 and if different, my residential address is: in the Kosciusko County, Indiana hereby gives notice that Bobby Darrell Lee Jr has filed a petition in the Kosciusko Circuit Court requesting that his name be changed to Damien Nelson Purley.
