    The Board of Public Works and Safety of the Civil City of Nappanee, Indiana, Elkhart and Kosciusko County, State of Indiana will receive sealed bids on Monday April 8, 2013 until the hour of 3:30 PM at the Clerk-Treasurer’s Office at the Nappanee Municipal Center for the purchase of two (2) new 12 Lead Monitor/Defibrillators, including battery packs and cables/parts.   Specifications for said two (2) new 12 Lead Monitor/Defibrillators, including battery packs and cable/parts, may be obtained at the office of the Clerk-Treasurer of the City of Nappanee, 300 West Lincoln Street, P.O. Box 29, Nappanee, IN 46550.  
    Total Bid shall reflect an amount allowed for trade-in of two (2) Zoll M-Series Monitor/ Defibrillators, which can be inspected weekdays from 8:00 AM through 3:00 PM, at 301 West Lincoln Street, Nappanee, IN 46550.
    All bids shall be submitted with a cash outright price minus trade-in.
    All bids shall be submitted on forms prescribed by the State Board of Accounts, which may be obtained at the office of the Clerk-Treasurer of said City and shall be accompanied by a non-collusion affidavit and bid bond, certified check or a corporate surety in a sum equal to five percent (5%) of the total bid.
    No bid shall be withdrawn after scheduled closing time for receipt of bids for at least ninety (90) days.  The Board reserves the right to reject any or all bids, to accept all, or any part of any bid received, and to waive any and all informalities in bidding, and to accept the lowest and/or best bid.
